Key Responsibilities

•        Functional Assessment: Conduct comprehensive evaluations of patients' ability to perform activities of daily living (ADLs), instrumental ADLs, and meaningful occupations in their home environment

•        Treatment Planning: Develop individualized intervention plans with measurable goals focused on maximizing independence in self-care, home management, and community participation

•        Therapeutic Interventions: Implement evidence-based occupational therapy techniques including ADL training, cognitive rehabilitation, upper extremity therapy, and adaptive equipment training

•        Home Modification Recommendations: Assess home environments and recommend adaptive equipment and modifications to enhance safety and independence

•        Patient & Caregiver Education: Provide expert guidance on energy conservation techniques, joint protection strategies, and compensatory techniques for daily activities

•        Documentation Excellence: Utilize WellSky Kinnser EMR and Andy AI technology for accurate, timely clinical documentation and OASIS assessments

•        Care Coordination: Collaborate effectively with physicians, nurses, other therapists, and healthcare team members to ensure comprehensive patient care

•        Progress Monitoring: Regularly reassess patient progress, adjust treatment plans as needed, and ensure adherence to clinical standards and regulatory requirements


Required QualificationsLicensure & Certifications

•        Graduate of an accredited Occupational Therapy program (MOT, OTD, or equivalent)

•        Valid Illinois Occupational Therapist license

•        NBCOT certification (or eligibility)

•        Current CPR certification

•        Valid driver's license and auto insurance
