Per-Diem Home Health Care Registered Nurse
Location: North Shore
Join our compassionate and dedicated team providing professional care to patients in the comfort of their homes. We are looking for a skilled Home Health Care Registered Nurse who is committed to patient-centered care.

Responsibilities
โ€ข Delivers skilled nursing care to patients in their home in accordance with individualized care plans.
โ€ข Initiates appropriate preventative and rehabilitative nursing procedures, making necessary referrals to community services.
โ€ข Coordinates services, informing the physician, management, and other personnel of changes in the patient's condition and needs.
โ€ข Ensures quality and accuracy of patient records in compliance with Medicare and Mass Health standards.
โ€ข Conducts ongoing medical documentation, writing OASIS assessments as appropriate.
โ€ข Prepares clinical and progress notes, submitting all required paperwork daily. This is a performance requirement.
