Pay: $54.00 - $56.00 per hour

Home Care Registered Nurse (RN) Responsibilities

  • up to 50 mile coverage
  • 30 units per week/ 4–6 patients/day/ 20–25 patients a week
  • Conducts comprehensive assessments of patient health and living conditions to develop personalized care plans and update them based on patient needs and progress.
  • Administers medications, wound care, injections, and other skilled nursing services following physician orders, maintaining strict adherence to safety and infection control standards.
  • Monitors patient conditions and vital signs, identifies signs of potential complications, and collaborates with primary care providers to adjust treatment plans as necessary.
  • Educates patients and families on managing health conditions, post-treatment care, and medication regimens, empowering them to take an active role in recovery and ongoing health maintenance.
  • Ensures accurate documentation of all patient care activities, updates electronic health records (EHR), and provides timely and detailed reports to the care team.
  • Coordinates with physical therapists, occupational therapists, and social workers to ensure holistic care and support tailored to patient needs.
  • Performs regular follow-up visits to monitor progress, address concerns, and modify care plans to meet evolving health and safety requirements


Home Care Registered Nurse (RN) Requirements:

  • Active RN Arizona State License
  • Valid driver’s license
  • Minimum 3 years Home Health experience required
  • Oasis experience required
  • Homecare Homebase required
