Job Descriptions for the position include:• Provides professional nursing care by utilizing all elements of the nursing process.
• Assesses and evaluates patient/client status by:
a.Reviewing and implementing the Plan of Care (POC)
b.Regularly re-evaluating patient/client needs
c.Revising the POC as necessary
• Administers medications and treatments as prescribed by the physician.
• Initiates and applies appropriate preventative, therapeutic, and rehabilitative nursing procedures and techniques and maintains communication regarding patient/client care with the physician and supervisory personnel.
• Regularly observe the patient/client for changes in condition, discuss changes with the patient/client or family/caregiver and reports changes, as necessary, to the attending physician.
• Assists the patient/client with the activities of daily living and facilitates the patients/clients effort toward self-sufficiency and independence.
• Teaches the patient/client and family/caregiver self care techniques as appropriate. Provides medication, diet, and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients/clients and families/caregivers.
• Provides and maintains a safe environment for the patient/client.
Qualifications:
• Currently licensed to practice as a RN in the state of practice.
• If IV or ET certified, is currently certified and recognized by the state(s) where currently practicing as an RN.
• Possesses excellent observation, good nursing judgment, and good oral and written communication skills.
• Possesses and maintains good physical stamina and mental health. Presents a pre-employment physician's health clearance, including negative TB skin tests and/or CXR and other tests as required by specific state statutes.
• Possesses and maintains current CPR certification.
• Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements.
Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.

This position is based on a Pay per visit compensation model.
Pay per visit ranges from $100-$150 visit commensurate with experience and depending on type of visit. The compensation reflected on this posting is an estimate of yearly compensation.
