Home Health Registered Nurse (RN) – Full-Time / Part-Time / Per Diem

Riverside County, CA (Menifee, Murrieta, Temecula, Hemet)

We are seeking a compassionate and experienced Home Health RN to join our growing team in Riverside County. This role offers the opportunity to provide one-on-one patient care in the home, with strong clinical support and a focus on efficient operations.

What You’ll Do

  1. Perform Start of Care (SOC), Resumption of Care (ROC), Recertifications, and routine skilled nursing visits
  2. Conduct patient assessments and develop individualized care plans
  3. Provide wound care, medication management, and disease education
  4. Monitor patient progress and communicate changes to physicians
  5. Complete timely and accurate documentation (OASIS/EMR)
  6. Collaborate with therapists, case managers, and interdisciplinary team

What Success Looks Like

  1. Average 2–6 visits per day (depending on visit type)
  2. Timely documentation and compliance with Medicare guidelines
  3. Strong communication with patients, families, and team members
  4. Consistent, high-quality patient care and outcomes

Qualifications

  1. Active California RN License (Required)
  2. 1+ year nursing experience (Home Health preferred)
  3. OASIS experience (preferred but not required)
  4. Strong assessment and critical thinking skills
  5. Valid driver’s license, reliable transportation, and auto insurance
  6. Current CPR certification
