Responsibilities:
  • Conduct comprehensive in-home patient assessments and develop individualized care plans
  • Administer medications, wound care, IV therapy, and other skilled nursing treatments
  • Monitor and document patient progress, vital signs, and changes in condition
  • Coordinate care with physicians, therapists, and other members of the interdisciplinary team
  • Educate patients and family members on disease management, medications, and home safety
  • Ensure compliance with Medicare, Medicaid, and home health regulatory standards
  • Maintain accurate and timely clinical documentation using electronic health records (EHR)
Requirements:
  • Active California Registered Nurse (RN) license in good standing
  • Minimum 1-2 years of clinical nursing experience; home health experience preferred
  • Strong knowledge of Medicare home health guidelines and OASIS documentation
  • Valid California driver's license, reliable transportation, and current auto insurance
  • Excellent clinical assessment, critical thinking, and time management skills
