What You'll Do

As a Home Health Registered Nurse, you'll provide skilled nursing care that helps patients safely recover and manage their health in the comfort of their own homes.

You'll:

  • Complete comprehensive patient assessments and develop individualized care plans.

  • Manage a designated patient caseload while coordinating care with physicians, therapists, and other healthcare professionals.

  • Perform skilled nursing visits and evaluate patient progress.

  • Educate patients and caregivers on medications, disease management, treatments, and preventive care.

  • Identify changes in patient condition and adjust the plan of care as appropriate.

  • Document care accurately and maintain compliance with all regulatory requirements.

  • Advocate for patients while promoting independence, safety, and quality outcomes.

What You'll Bring

Required Qualifications

  • Current, unrestricted Texas Registered Nurse (RN) license

  • Minimum of one (1) year of nursing experience in an acute care, skilled nursing, or clinical setting

  • Excellent assessment, communication, and critical thinking skills

  • Reliable transportation, valid driver's license, and current auto insurance

  • Willingness to travel throughout the assigned coverage area

  • Physical Requirements: Sit, stand, bend, lift, and move intermittently throughout the workday. Must be able to lift, push, pull, and support up to 50-100 pounds, including assisting with patient positioning, transfers, and moving equipment.

Preferred Qualifications

  • Home Health or Hospice experience

  • Experience completing OASIS assessments

  • Homecare Homebase (HCHB) experience

  • Associate or Bachelor's Degree in Nursing
