What are some common challenges Home Health RNs face when managing patient care independently in the field?

Home Health RNs often encounter challenges such as adapting to diverse home environments, managing time efficiently between multiple patient visits, and addressing unexpected changes in patient conditions without immediate on-site support. Effective communication with physicians, family members, and interdisciplinary team members is crucial for coordinating care and ensuring patient safety. Additionally, Home Health RNs must be comfortable with technology for electronic health records and telehealth, as well as self-motivated to stay updated on best practices, since they work with a high degree of autonomy.

What does a Home Health RN do?

The job duties of a home health RN involve providing care for a patient in their house. You often work with elderly patients, people with illnesses or chronic conditions, and those recovering from surgery. As a home health RN, your duties include taking the patient’s vital signs, ensuring they manage their medication and treatments correctly, and making a general assessment of their condition. Your responsibilities can include connecting IVs, giving shots, and cleaning and examining wounds. You coordinate with a physician or other staff members to ensure that you follow the plan of care and meet each patient's healthcare needs.

What are the key skills and qualifications needed to thrive as a Home Health RN, and why are they important?

To thrive as a Home Health RN, you need strong clinical assessment skills, knowledge of home care protocols, and an active RN license, often supported by experience in community or acute care nursing. Familiarity with electronic health record (EHR) systems, remote patient monitoring devices, and sometimes certifications like BLS or OASIS are important. Exceptional time management, independence, and compassionate communication are crucial soft skills for building trust with patients in their homes. These abilities are vital to ensure safe, effective, and patient-centered care in diverse and often unsupervised home environments.

This role requires strong clinical skills, independence, and effective communication to ensure patients receive quality care at home.

Job description

RN Pediatric Clinical Manager— Hybrid

St. Cloud Pediatrics (SCP) Office | Minnesota

Employment type: Full time

Salary: $80,000–$90,000 per year, paid weekly

Service area: St. Louis County

Work arrangement: Hybrid home-and-field role; significant field presence required

This position supports clients and clinicians across the St. Louis County area. Relocation to the service area is required for this role.

This is a hybrid home-and-field position with significant travel throughout St. Louis County to support clients, clinicians, and care delivery. This is not a fully remote position.

Join BAYADA in a leadership role that makes a difference

Are you a Registered Nurse licensed in Minnesota who is ready to lead clinical teams and make a meaningful difference for children and families? Do you have experience supervising, mentoring, or developing clinicians in a home care, pediatric, or skilled nursing setting? Are you looking for a flexible schedule that combines hands-on field leadership with focused work-from-home time?

BAYADA Home Health Care is seeking an RN Pediatric Clinical Manager for the St. Cloud Pediatrics (SCP) office in Minnesota. The office provides adult and pediatric skilled nursing services as well as pediatric personal care services.

In this role, you will help ensure that medically fragile clients receive safe, consistent, and compassionate care in their homes. You will serve as a clinical resource for field employees, a trusted partner to clients and families, and a key member of the office leadership team. Your leadership will help strengthen continuity of care, support clinician success, and uphold BAYADA's professional, ethical, and safety standards.

Our Skilled Nursing practice provides expert, compassionate care to medically fragile children and adults in the home. Care may include complex needs such as tracheostomy and ventilator support, seizure management, and tube feedings.

Position summary

The Clinical Manager leads field staff in delivering excellent client service, clinical care, quality, and adherence to policies and procedures. The role combines clinical leadership, case management, staff development, client and family support, and operational partnership.

The successful candidate will be comfortable moving between the field and home-based work. Some days may focus on client visits, assessments, supervisory visits, clinician coaching, or care coordination. Other days may include documentation, case review, care-plan updates, staffing collaboration, and follow-up with clients, families, clinicians, and office leadership.

What the hybrid role looks like

Hybrid flexibility is built into the position to support both responsive clinical leadership and efficient administrative work. The role requires regular in-person engagement and travel throughout the service area.

In the field

  • Visit clients in their homes to conduct assessments, evaluate care needs, and support the care planning process.
  • Complete supervisory and quality visits with field clinicians.
  • Observe care delivery and provide real-time coaching, education, and clinical guidance.
  • Build trusting relationships with clients, families, caregivers, and clinicians.
  • Support continuity of care for pediatric and adult skilled clients.
  • Respond to changing client needs and help address clinical or service concerns.
  • Travel throughout St. Louis County as needed to support safe, high-quality care delivery.

From home

  • Complete clinical documentation, care plans, assessments, and follow-up activities.
  • Review field employee documentation and clinical performance.
  • Coordinate cases, staffing needs, client services, and interdisciplinary communication.
  • Prepare for client visits and follow up on clinical recommendations.
  • Coach and mentor clinicians through phone, video, and other communication channels.
  • Partner with office leadership on quality, compliance, retention, and service delivery priorities.

Qualifications

  • Current Registered Nurse license in good standing in Minnesota required.
  • Minimum of two years of nursing experience preferred; home health experience may vary based on office need.
  • Experience supervising, mentoring, or developing clinicians in a home care, pediatric, or skilled nursing setting strongly preferred.
  • Trach, ventilator, and tube-feeding experience in a home health care setting preferred.
  • Demonstrated leadership, organization, multitasking, communication, and problem-solving skills.
  • Ability to work independently in a field-based environment while remaining connected to the office and care team.
  • Willingness and ability to travel regularly throughout the assigned service area.
  • Commitment to professional, ethical, and safe clinical practice.
  • Ability to relocate to the service area is required if far away.

Schedule and work environment

  • Full-time position with a flexible schedule.
  • Hybrid home-and-field role with the majority of time spent in the field.
  • Regular travel throughout St. Louis County to support clients, clinicians, and care delivery.
  • Work-from-home time is built in for documentation, care coordination, case review, and follow-up.
  • The role requires reliable communication, strong time management, and the ability to adjust priorities as client and staff needs change.
  • Participation in after-hours clinical consultation and on-call support as needed.

Why you'll love BAYADA

  • Flexible work schedule.
  • $80,000–$90,000 annual salary, paid weekly.
  • Growth opportunities.
  • Comprehensive benefits package, including paid holidays, vacation and sick leave, medical, dental, and vision coverage, employer-paid life insurance, 401(k) with company match, direct deposit, and an employee assistance program.
  • The stability and structure of a national organization with the values and culture of a family-owned business.
