Activa Home Health, a licensed, Medicare-certified home health agency, is seeking experienced and compassionate Field Registered Nurses to join our team.

Position Summary
The Field RN provides skilled nursing care to Medicare home health patients in accordance with each patient's individualized Plan of Care, with a strong focus on safety, clinical outcomes, and patient satisfaction.
Experience with WellSky Kinnser, OASIS, and Medicare home health is required.
Key Responsibilities
  • Provide safe, effective, and patient-centered nursing care in accordance with the individualized Plan of Care during Medicare home health visits
  • Accurately complete and document all visits in the EMR, including Start of Care and ongoing assessments using the OASIS tool
  • Deliver skilled nursing services within scope of practice, including:
  • Medication administration
  • Wound care
  • infusion therapy
  • Catheter care
  • Safe transferring and positioning of patients
  • Perform all required supervisory visits as assigned
  • Communicate promptly with the interdisciplinary care team regarding any changes in patient condition, concerns, or emergencies
  • Collaborate with the Nurse Supervisor and Manager of Clinical Practice to ensure continuity and quality of care
  • Maintain professionalism, empathy, attention to detail, and strong clinical judgment in the home health setting

Required Qualifications
  • Active Florida Registered Nurse license
  • Medicare home health experience required
  • OASIS experience required
  • Wound care experience required
  • Proficiency with electronic medical record systems (WellSky Kinnser preferred)
  • Minimum of 1 year of home health experience
  • Strong communication, teamwork, and customer service skills
  • NEGATIVE TB Test / NEGATIVE QuantiFERON Blood Test
