Physical Therapist – Home Health

Position Summary

As a Physical Therapist, you will assess, plan, and deliver individualized therapy programs designed to improve mobility, relieve pain, restore function, and enhance overall patient well-being. You'll work closely with patients, families, and interdisciplinary care teams to provide exceptional home health care services.

What You'll Do
  • Perform and document comprehensive patient evaluations and establish treatment plans
  • Develop and implement individualized therapy programs to restore physical function and mobility
  • Monitor patient progress and modify treatment plans to maximize outcomes
  • Educate patients and caregivers on exercises, safety, and continued care strategies
  • Maintain timely and accurate clinical documentation
  • Collaborate with physicians, nurses, and healthcare professionals to ensure continuity of care
  • Follow infection control and universal precaution guidelines
  • Support the agency's mission, values, and patient-centered care philosophy
Qualifications
  • Graduate of an accredited Physical Therapy program
  • Current Physical Therapy license
  • CPR Certification
  • Valid driver's license and auto insurance
  • Home Health experience preferred
  • HomeCare HomeBase experience preferred
Physical Requirements
  • Ability to stand, walk, bend, kneel, squat, and move throughout the day
  • Ability to lift, carry, push, or pull up to 100 lbs.
