Responsibilities:

Manage and monitor peritoneal dialysis (PD) treatments, including CAPD and APD modalities

Educate patients and caregivers on PD procedures, infection prevention, and home dialysis techniques

Assess patients for complications such as peritonitis, catheter infections, and fluid imbalances

Collaborate with nephrologists and interdisciplinary care teams to develop and adjust individualized treatment plans

Document patient progress, treatment outcomes, and clinical notes accurately in EMR systems

Conduct follow-up visits and telehealth check-ins to ensure patient compliance and well-being

Maintain PD supplies, equipment, and inventory in accordance with clinical protocols

Requirements:

Active RN license in the state of Texas (required)

Prior experience in peritoneal dialysis, hemodialysis, or nephrology nursing preferred

CNN (Certified Nephrology Nurse) certification is a plus

Strong patient education and communication skills
