Aging Services of North Central Massachusetts (ASNCM) is seeking a Community Case Manager – Older Adult Services to join our State Home Care team.


This position is ideal for professionals who enjoy building meaningful relationships, working independently, connecting consumers with community resources, and helping older adults remain safely in their homes.


Rather than sitting behind a desk all day, you will work directly with consumers, family members, caregivers, healthcare providers, and community partners to assess needs, develop service plans, and coordinate services that improve quality of life and support independent living. The State Home Care Case Manager role includes assessing needs, developing service plans, coordinating resources, monitoring progress, and maintaining program documentation.

What You'll Do

As a Community Case Manager, every day is different. You may:

  • Conduct assessments to evaluate physical, social, environmental, and emotional needs.
  • Develop individualized service plans that support safe and independent living.
  • Meet with consumers and caregivers to identify goals and service needs.
  • Connect consumers with community resources and support services including housing assistance, fuel assistance, healthcare services, Medicaid-related programs, and other community supports.
  • Coordinate services with family members, caregivers, healthcare providers, and community agencies.
  • Monitor consumer needs and adjust service plans as circumstances change.
  • Advocate for consumers and help them navigate complex systems.
  • Maintain accurate documentation, authorizations, notices, assessments, and monthly reports.

Required Qualifications

  • Bachelor's  degree or equivalent experience.
