Pay range (depending on experience):ย 
Visit Rate: $60.00ย 
Start of Care Rate: $120.00ย 
Resumption of Care Rate: $120.00ย 
Recertification Rate: $115.00ย 
Evaluation Rate: $75.00ย 
Discharge OASIS: $75.00ย 
Discharge: $60.00ย 
Non-Visit Activity/Hourly: $40.00
ย 
Schedule/Shift: Mon-Fri 8am-5pm
ย 
Territory/Location: West Valley: Buckeye, Avondale, Goodyear, Litchfield park, Sun City, Surprise
Responsibilities:
  • Completes an initial assessment of the patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).ย 
  • Initiates the plan of care and makes necessary revisions as patient status and needs change.ย 
  • Regularly re-evaluates patients' needs.ย 
  • Uses health assessment data to determine nursing diagnosis.ย 
  • Develops a care plan which establishes goals, based on nursing diagnosis, incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.ย 
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.ย 
  • Counsels the patient and family in meeting plan of care and related needs.ย 
  • Provides health care instructions to the patient as appropriate per assessment and plan.ย 
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.ย 
Qualifications:
  • Current and valid state RN license.ย 
  • Minimum of 1 year of recent acute care experience in an institutional setting.ย 
  • Basic Life Support (BLS), with hands-on CPR certification.ย 
  • Valid driver's license and auto insurance.ย 
  • Associate's or Bachelor's degree in Nursing, and 1 year of home health experience preferred.ย
