Key Responsibilities

  • Assist patients in developing personalized rehabilitation goals and monitor their progress through meticulous tracking and documentation.
  • Hold regular reassessment sessions to modify treatment plans based on patient progress and changing needs.
  • Educate patients, families, and caregivers on the usage of assistive devices and physical therapy procedures to promote independence and self-care capabilities.

Patient Population / Client Focus

Your patient base will span a diverse demographic, encompassing individuals of all ages recovering from injuries, surgery, or chronic conditions. The home care aspect of this role allows you to foster a trusting relationship with patients, enhancing their comfort and compliance with prescribed treatment, which is pivotal for successful recovery and rehabilitation.

Qualifications
  • A Doctorate Degree in Physical Therapy (DPT) is required to ensure a deep understanding of advanced therapeutic techniques.
  • Must hold a valid State License in Physical Therapy, confirming your eligibility to practice in Illinois.
  • Strong written and verbal communication skills are essential for effectively conveying treatment plans and instructions to patients and families.
