• Conduct thorough clinical assessments using telehealth platforms, ensuring a detailed understanding of each patient’s psychosocial needs.
• Develop and implement personalized treatment plans incorporating evidence-based practices such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), adjusted for virtual delivery.
• Create and maintain accurate clinical records in secure electronic health record (EHR) systems, in strict compliance with HIPAA and other regulatory guidelines.
• Engage in regular collaboration with a multidisciplinary treatment team—including psychiatrists, psychologists, and other social workers—facilitated through virtual meetings.
• Provide educational resources and support to patients and their families regarding mental health issues, treatment options, and community resources.
• Utilize innovative digital resources and participate in ongoing training, webinars, and professional development opportunities to stay current in the field of social work.

The patient population served in this role primarily includes individuals and families navigating end-of-life issues, requiring a sensitive and nuanced approach to mental health care. You will support patients who are coping with grief, loss, and transitions in care while helping them and their families understand and access available resources.

• A Master’s degree in Social Work from an accredited institution.
• Must hold a valid LCSW license in the state of Pennsylvania.
• Minimum of 2 years of clinical experience with a proven track record in providing psychotherapy services.
• Expertise in solution-focused therapy interventions suitable for a remote setting.
• Exceptional communication skills necessary for establishing therapeutic rapport with clients in virtual environments.
• Strong organizational skills and proficiency in managing multiple caseloads efficiently.

• Experience in hospice care or working with patients facing terminal illnesses is highly desirable.
• Familiarity with telehealth technology and experience delivering remote therapy effectively.
• Knowledge of community resources and support organizations relevant to hospice care.
