Job description

A private family in Flushing, New York is seeking an experienced Registered Nurse to provide dedicated clinical support, care coordination, and day-to-day health oversight for an 84-year-old male principal in a private home setting.

The principal has diabetes and late-stage kidney disease requiring dialysis, including home dialysis support. This role is ideal for a seasoned, compassionate nurse who brings strong clinical judgment, discretion, consistency, and professionalism to a highly personalized care environment. The RN will serve as a trusted health partner, helping to support medical needs, daily routines, provider coordination, and overall well-being.
This is a full-time position (40 hours per week), ideal for a nurse who is confident operating independently, highly detail-oriented, and experienced managing chronic conditions within a private or concierge setting.
This is intended to be a dedicated weekday role, with flexibility based on appointments, treatments, changing medical needs, and occasional travel. The family is open to discussing the exact schedule with the right candidate.
Mandarin or Taiwanese language skills are preferred.


  • Provide ongoing nursing support and health oversight in a private home environment.
  • Monitor daily health status, symptoms, vital signs, and overall condition, and communicate changes promptly and appropriately.
  • Support diabetes management, dialysis care including home dialysis, medication oversight, and adherence to physician-directed care plans.
  • Coordinate medical appointments, treatments, specialist follow-up, and communication across providers to support continuity of care.
  • Attend medical appointments as needed and help track recommendations, care changes, and next steps.
  • Assist with activities of daily living and personal care as needed, including hygiene-related support, with professionalism, discretion, and respect.
  • Support physician-recommended nutrition, hydration, exercise, and physical activity plans.
  • Assist with light meal preparation aligned with diabetic and renal dietary needs.
  • Provide transportation to medical appointments, treatments, errands, and approved activities as needed.
  • Maintain organized documentation related to medications, appointments, provider guidance, and ongoing care needs.
  • Recognize changes in condition and respond calmly and appropriately in urgent or time-sensitive situations.
  • Support occasional travel and help maintain continuity of care across locations when needed.

  • Meaningful experience caring for older adults and individuals with complex medical needs, ideally including diabetes, kidney disease, and dialysis.
  • Experience in private duty, home-based care, geriatric care, or high-touch care coordination environments preferred.
  • Comfortable providing hands-on support with personal care and hygiene as needed.
  • Strong communication skills and the ability to work closely with family members, physicians, and other caregivers.
  • Highly organized, dependable, proactive, and discreet.
