Key Responsibilities

As the Licensed Clinical Social Worker, your daily activities will encompass a variety of essential tasks designed to support the comprehensive care of our patients. Key responsibilities include:

• Conducting thorough psychosocial assessments to evaluate patients' emotional, social, and financial needs in the context of detoxification and recovery.
• Providing individual and group counseling that addresses adjustment, grief, substance abuse issues, and coping strategies, facilitating understanding and healing.
• Developing discharge plans in collaboration with interdisciplinary teams, ensuring seamless transitions to appropriate community resources upon patient discharge.
• Facilitating communication between patients, families, and healthcare professionals, offering education on treatment options and available support resources.
• Coordinating with community organizations and services to secure follow-up care, thereby promoting sustained recovery and improving long-term outcomes.
• Providing crisis intervention support as necessary to patients and their families, helping them navigate challenging situations effectively and empathetically.
• Maintaining accurate records and documentation that meet hospital policies and legal requirements to ensure compliance and continuity of care.

Patient Population / Client Focus

In this role, you will specifically serve individuals undergoing detoxification from various substances, including alcohol and drugs. This patient population often experiences complex psychosocial challenges and requires empathetic and effective support mechanisms. Qualifications

The ideal candidate will possess the following qualifications:

• A Master’s degree in Social Work from an accredited institution.

Preferred Experience
