Key Responsibilities
  • Perform ongoing assessments to accurately evaluate and address the individualized needs of clients.
  • Effectively communicate with supervisors, family members, and other stakeholders involved in a client’s progress, ensuring a comprehensive support system.
  • Develop and propose tentative schedules to supervisors for client appointments, home visits, and observations, facilitating effective monitoring of progress.
  • Coordinate and conduct home visits as required, ensuring thorough in-home observations, chart reviews, and staff interviews.
  • Monitor and evaluate the implementation of counseling program plans to ensure they are executed effectively, making adjustments as necessary.
  • Actively participate in Interdisciplinary Team Meetings and additional relevant meetings to enhance collaborative care approaches.
  • Document monthly and quarterly progress notes to ensure consistent records of client development and treatment effectiveness.
  • Organize and lead training sessions to ensure staff and team members understand program plans and documentation processes.
  • Conduct follow-up interviews and home visits as part of ongoing client support to address evolving needs.
  • Serve as a liaison with the public and mental health communities to foster supportive relationships and enhance program development.
  • Support the creation of effective training materials aimed at improving treatment and services delivered to clients.
Patient Population / Client Focus

Qualifications

  • Strong understanding of legal and ethical standards relevant to counseling practices.

Compensation and Benefits
