About the Opportunity
A mission-driven, physician-owned organization is expanding its care model and seeking a Primary Care Nurse Practitioner to deliver personalized care to adult and geriatric patients in home and facility-based settings across the Greater Cleveland area.
This is not your typical clinic role. You'll enjoy full autonomy, a flexible schedule, and a collaborative support system that includes clinical leadership from a nationally recognized DNP. You'll provide general health maintenance and chronic disease management to patients already being served through a wound care program, adding value and continuity to their care journey.

What You'll Do

  • Deliver in-home and facility-based primary care to adult and geriatric patients
  • Diagnose and manage chronic and acute conditions with a holistic lens
  • Coordinate care with nurses, ADONs, DONs, and clinical teams across care settings
  • Maintain timely documentation and collaborate with leadership on care protocols
  • Educate patients and families on disease prevention, medication adherence, and long-term wellness strategies
  • Participate in quality initiatives and leverage tech-enabled tools for care efficiency
What You'll Need
  • Active Nurse Practitioner license in Ohio (MSN or DNP)
  • National board certification (AANP or ANCC)
  • Active DEA license + BLS/CPR certification
  • Minimum 2 years of experience in adult/geriatric primary care or home-based care
  • Strong clinical decision-making skills and comfort working independently
  • Empathy, professionalism, and a team-oriented mindset
  • Reliable transportation and ability to travel locally within the Cleveland metro
