Job description

Parx Home Health Care is looking for a Registered Nurse to join our team.  

The home health nurse will be responsible for providing high quality and coordinated care to patients in their homes and communities. The ideal candidate will  have a strong background in home health care, an understanding of state and federal regulations, and a passion for helping patients live their best lives.

What you’ll be doing:

  • Conduct comprehensive assessments of patients' health status, including physical, emotional, and social factors.
  • Develop and implement individualized care plans that are tailored to each patient's needs and goals.
  • Provide patient education and support to promote self-management and prevent complications.
  • Coordinate care with other healthcare providers, including physicians, therapists, and social workers, to ensure continuity of care.
  • Administer medications, treatments, and procedures as ordered by the physician or authorized healthcare provider.
  • Document all patient care activities and communicate patient status and progress to the healthcare team.
  • Participate in quality improvement activities and initiatives to improve patient outcomes and satisfaction.
  • Maintain professional knowledge and skills by attending continuing education programs and staying up to date with the latest developments in the field.
  • Provide emotional support and compassionate care to patients and their families
  • Adhere to all applicable laws, regulations, and ethical standards of nursing practice

Requirements

What we expect from you:

  • Current and unrestricted RN license in the state of Florida.
  • Strong clinical skills and ability to perform evidenced based nursing practices. 
  • 1-3 years of clinical experience in a healthcare setting, preferably in home health, hospice, or community health.
  • Excellent communication and interpersonal skills with the ability to work effectively with patients, families, and healthcare providers.
  • Physical Exam and Two-Step TB Test or Chest X-Ray (no older than 6 months).
  • Yearly Mandatory in-service.
  • Proficient in using electronic health records and other healthcare technologies.
  • Valid FL driver's license and reliable transportation (Auto Insurance must also be presented)
  • CPR certification required (Original Card)
  • Level 2 Criminal Background Check
