Job description

Flexible Schedule • Independent Contractor • Adult & Geriatric Patients

Help Patients Stay Independent at Home.

@Home Therapy is seeking a licensed Occupational Therapist (OT) to provide home-based occupational therapy services to adult and geriatric patients throughout Brooklyn.
This is an excellent opportunity for therapists who enjoy one-on-one patient care, flexible scheduling, and helping patients maximize independence in their own homes.

What You'll Do:
  • Perform occupational therapy evaluations in the home.
  • Assess ADLs, IADLs, upper extremity function, cognition, coordination, home safety, and functional independence.
  • Develop individualized treatment plans to improve daily function and quality of life.
  • Train patients and caregivers in adaptive equipment, energy conservation, fall prevention, and safe performance of everyday activities.
  • Monitor patient progress and modify treatment plans as needed.
  • Complete timely EMR documentation and collaborate with physicians and the interdisciplinary care team.

Qualifications:
