Job description

Build Relationships. Coordinate Care. Help Patients Transition Home Safely.
PurposeCare is seeking a relationship-driven and operationally strong Hospital Liaison to support hospital partnerships and help patients transition smoothly into home health services.
This role is ideal for someone who thrives in a fast-paced healthcare environment, enjoys building strong relationships with hospital teams, and understands the importance of urgency, communication, and patient advocacy during care transitions.
As a key member of our team, you'll work directly with hospital case managers, discharge planners, physicians, patients, and families to coordinate referrals, support admissions, and improve continuity of care.
What You'll Do
  • Coordinate home health referrals and patient transitions from hospitals into home-based care
  • Partner closely with hospital case managers, discharge planners, social workers, and healthcare teams
  • Manage and follow up on referrals with urgency and strong attention to detail
  • Educate patients and families on home health services and available support options
  • Conduct regular onsite hospital visits and maintain strong referral relationships
  • Collaborate with PurposeCare Intake and Clinical Operations teams to support timely Starts of Care
  • Utilize systems such as:
    • CarePort
    • WellSky
    • Axxess
    • Trella
  • Help identify opportunities to improve referral conversion and patient experience
  • Support awareness of both home health and private pay service offerings
What We're Looking For
  • RN, LPN, PT, OT, or MSW required
  • Minimum of 3 years of experience in:
    • Home health
    • Care transitions
    • Clinical liaison work
    • Hospital case management
    • Healthcare operations
  • Strong relationship-building and customer service skills
  • Ability to manage multiple priorities in a high-volume healthcare environment
  • Strong attention to detail and follow-through
  • Experience working with referral platforms and EMR systems preferred
  • Strong critical thinking and communication skills
  • Comfortable working independently in the field while collaborating closely with operational teams
