These nurses use telehealth technology to assess, monitor, and support patients, often coordinating care with physicians and other healthcare professionals. Familiarity with remote patient monitoring systems, electronic health records (EHRs), and Kaiser Permanente's clinical protocols is essential. Exceptional communication, self-motivation, and organizational skills help nurses effectively coordinate care and support patients independently. These competencies ensure safe, high-quality care delivery while meeting the unique needs of patients in a home setting.
