Home Health Registered Nurse (RN)

Providence at Home is a growing division of Providence Life Services, which provides a continuum of healthcare, focused on senior living. Providence at Home is seeking a Home Health Registered Nurse (RN), Full Time in the Oak Lawn, IL area as well as in Burbank, Tinley Park, Orland Park, Markham, Justice, Oak Forest and Midlothian, IL. Providence at Home is a Joint Commission accredited and celebrating 30 years of home-based services.

Schedule Available: Monday - Friday during normal business hours. On-call duties receive additional compensation. Home Health Visits happen in a 25 mile radius within Oak Lawn, IL.

Summary of essential job functions:

  • Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(s).
  • Regularly re-evaluates patient nursing needs.
  • Initiates the plan of care and makes necessary revisions as patient status and needs change.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures.

Requirements:

  • Must be a Licensed Registered Nurse (RN) for the State of IL.
  • Minimum of 1-2 years' experience, at least one of which in the area of home care nursing, hospice, ER or Med surg.
  • Must be CPR certified.
  • Driver's License and good driving record.
  • Reliable transportation to travel to patients in the community.
