This role is important for patients who require ongoing medical attention but prefer to remain at home, such as those recovering from surgery, managing chronic illnesses, or needing palliative care. Job description

Internal Medicine NP/PA
Home-Based Primary Care

Four-Day Work Week | No Call | Physician-Owned Practice

Old-Fashioned House Calls. Modern Internal Medicine.

Medicine has changed dramatically over the last century.
One thing shouldn't have.
We still believe the best medicine happens when providers truly know their patients.

Comprehensive Medicine is a physician-owned Internal Medicine practice providing home-based primary care to medically complex patients throughout San Diego County. We are not a Home Health agency. We are not hospice. We are not a nursing service.
We are the patient's primary care provider.
Our office simply happens to be wherever the patient calls home.
That may be a private residence, an independent living community, an assisted living facility, or a residential care home. Regardless of where they live, our mission is the same:
Keep patients healthy, independent, and out of the emergency room whenever safely possible.

Why We Exist

Years ago, after caring for countless elderly patients in the Emergency Department, we became frustrated watching the same cycle repeat itself.
Emergency room.
Hospital.
Rehabilitation.
Home.
Back to the emergency room.
Often the patient's primary care provider hadn't seen them in months. Nobody truly knew the patient, coordinated their care, or recognized problems before they became emergencies.
We built Comprehensive Medicine to change that.
Instead of waiting for patients to become sick enough for another hospitalization, we bring comprehensive Internal Medicine directly to them.

This Is Real Continuity of Care

Our providers don't just treat today's complaint.
They become the clinician who knows the patient.
You'll follow patients over months and years, coordinating their care with specialists, hospitals, skilled nursing facilities, Home Health agencies, hospice, therapists, family members, pharmacies, and mobile diagnostic services.
You'll order and interpret laboratory testing, portable X-rays, ultrasound, EKGs, and other diagnostics without requiring many patients to leave their homes.
You'll learn how every part of the healthcare system works together and, more importantly, how to keep patients from falling through the cracks.

Company Description

“We didn’t build this practice because house calls are nostalgic. We built it because too many patients were cycling through emergency rooms, hospitals, rehabilitation facilities, and back home with no one truly coordinating their care. We believed there had to be a better way. We still do.”
Brian Peterson, PA & Dr. Mary Gessner-Peterson
