Job description

The Staff Pad has partnered with a growing, mission-driven healthcare organization in Lawrence, MA to recruit a Geriatric Nurse Practitioner for its primary care practice.

This is an opportunity to join an innovative practice focused on delivering exceptional care to older adults while enjoying real schedule flexibility and the ability to practice at the top of your license.

You'll provide comprehensive care to approximately 1,600 Medicare patients, many with complex healthcare needs, across the clinic, patients' homes, and senior living communities.

Schedule: Full-Time | Flexible Scheduling

What You’ll Do

  • Assess, diagnose, treat, and manage acute and chronic conditions in older adults
  • Lead Medicare Annual Wellness Visits and close preventive care gaps
  • Prescribe and reconcile medications, and educate patients and caregivers
  • Provide care management for high-risk and recently hospitalized patients
  • Guide advance care planning and goals-of-care conversations
  • Conduct home and facility visits across office, home, and senior living settings

What We Are Looking For

  • MSN or DNP from an accredited Nurse Practitioner program
  • Active Massachusetts NP/APRN license, with the ability to obtain New Hampshire licensure
  • Board certification as an Adult-Gerontology, Gerontological, or Family NP (ANCC or AANP)
  • Prescriptive authority and DEA registration, or eligibility to obtain
  • Extensive experience caring for geriatric patients
  • Valid driver's license and reliable transportation for home and facility visits
