This Home Health Physical Therapist position is based in Medford, Oregon, a welcoming city in the heart of Southern Oregon. In this role, you will provide home-based physical therapy services to patients across Medford and the surrounding Southern Oregon area. You will travel to patient residences, conduct thorough evaluations, develop individualized treatment plans, and deliver one-on-one interventions designed to maximize safety, mobility, and independence in the home setting.
