Work Environment

This role is primarily onsite, requiring travel to patients' homes in the Winter Haven area. Our home care facility facilitates an intimate and supportive environment where therapists can interact directly with patients and their families. You will benefit from a flexible work schedule, operating primarily Monday through Friday, allowing for a healthy work-life balance.

Key Responsibilities

• Evaluate and assess patients requiring physical therapy services due to injuries, surgeries, or chronic conditions within a home care context.
• Develop, implement, and adjust tailored treatment plans using evidence-based practices and a variety of therapeutic techniques, including exercises, stretching routines, and manual therapy.
• Monitor patient progress meticulously and document treatment outcomes to optimize future care strategies.
• Educate patients and their families on therapy procedures and the use of assistive devices to promote independence and self-care.
• Maintain accurate records and ensure compliance with health regulations while collaborating with other healthcare professionals.
• Stay informed about the latest developments in physical therapy by attending workshops, seminars, and professional development opportunities.

Patient Population / Client Focus

Our therapists cater to a diverse client base, including infants, children, adults, and seniors, dealing with a wide range of diagnoses. You will work closely with patients post-surgery, those recovering from injuries, or individuals with chronic disabilities, ensuring that each treatment plan is as unique as the patient themselves.

Qualifications

• A Bachelor’s Degree in Physical Therapy from an accredited institution is required.
• Current state licensure as a Physical Therapist in Florida.
• Excellent communication skills (both written and verbal) are essential for effective patient interactions and documentation.
• Proficient in word processing and computer skills, necessary for documentation and communication.
• A Doctorate of Physical Therapy (DPT) is preferred but not mandatory.
• Current CPR certification is required.

Preferred Experience

• At least two years of relevant experience in a clinical or home care setting is highly desirable.
• Experience working with pediatric and geriatric populations is an advantage.
• Demonstrable ability to create and implement comprehensive treatment plans tailored to individual needs.

Compensation and Benefits
