Position Overview

Equis Care Psychological Services is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our innovative team. Key Responsibilities
  • Conduct thorough psychosocial assessments through structured interviews, observation, and comprehensive reviews of medical records to evaluate the emotional, social, and environmental factors influencing recovery.
  • Provide empathetic counseling and psychoeducation for patients and their families, facilitating adjustment to new realities and enhancing overall emotional resilience.
  • Collaborate closely with diverse rehabilitation professionals to craft individualized treatment plans, aligning with each patient's emotional and social needs.
  • Assist patients in navigating community resources, such as connecting them with support groups, vocational training, or housing assistance, ensuring a continuum of care after rehabilitation.
  • Actively participate in discharge planning, guiding patients as they transition back to their homes or other necessary care settings.
  • Maintain accurate documentation of patient progress and interventions in compliance with regulatory standards and organizational policies.
Patient Population / Client Focus

Our clinic serves a diverse patient demographic, including individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and chronic neurological disorders. You will engage with patients across varied backgrounds, addressing not only their mental health but also understanding the social determinants affecting their recovery. Qualifications
  • A Masterโ€™s degree in Social Work from an accredited program is required to ensure a strong foundation in clinical practices.
  • An active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory to practice in Colorado.
  • Exceptional communication and interpersonal skills to build rapport with patients and collaborate with interdisciplinary teams.
Preferred Experience

Compensation and Benefits
