Join our team and make a positive impact on the lives of others! We are looking for a Field Onboarding Specialist I for our team in Colorado.
This is a field-based role with required fieldwork through the Denver, CO area.
The Field Onboarding Specialist is a field-based role where you will travel through Central Colorado meetings with patients and caregivers in their homes. This role supports the Intake team ensuring the patient and caregiver in providing education, submitting required documents, and brief orientation to ensure coordination for start of care.
Every day you will:
• Visit patients and caregivers in their homes to support the enrollment process to start of care.
• Act as primary point of contact supporting the completion of eligibility and employment paperwork for the caregiver.
• Demonstrate effective case management by maintaining a 3-hour email, chatter and voicemail response time.
• Achieve monthly standard to enroll a minimum 40% of caregivers based on state standards.
• Navigate multiple systems as required to complete the enrollment process.
• Maintain detailed case notes in Salesforce.
• Adhere to HIPAA guidelines and compliance expectations.
• Other duties as assigned by management
Education / Experience
  • 1+ years' experience in field-based role •
  • Excellent written and verbal communication skills •
  • Your own reliable transportation and ability to travel as required •
  • A self-starter personality and the ability to work independently with minimal supervision •
  • Ability and discipline to work remotely as required •
  • Bilingual Spanish nice to have
