This position is primarily home-based, allowing our therapists to provide care in the comfortable and familiar surroundings of patients' residences. Operating within a flexible Monday to Friday schedule from 9 AM to 5 PM, our physical therapists benefit from a positive work-life balance. The role requires travel to various patient locations within the Ridgewood area, and therapists will utilize their own reliable transportation to reach clients directly. Collaboration with our administrative staff and coordination with healthcare professionals is also integral to the role, enhancing the continuity of patient care.

• Conduct comprehensive evaluations of patients' physical capabilities and limitations through detailed interviews, observations, and thorough physical assessments.
• Design individualized physical therapy plans that reflect each patient's unique needs, rehabilitation goals, and clinical best practices.
• Implement hands-on therapeutic interventions, including but not limited to exercise regimens, manual therapy techniques, gait and balance training, to optimize patient progress.
• Provide education to patients and their families on understanding the recovery journey, managing pain effectively, and continuing rehabilitation exercises beyond therapy sessions.
• Maintain meticulous patient records, documenting evaluations, treatment plans, progress, and outcomes as per company policies and regulatory standards.
• Regularly assess patient progress, modifying treatment plans as needed and communicating any significant changes in condition to physicians and family members.

The Home-Based Physical Therapist will engage with a diverse patient population, primarily consisting of individuals recovering from surgeries, injuries, or chronic conditions. These patients include children, adults, and seniors who may have limited mobility or specific rehabilitation needs. Your role is critical in helping patients achieve their functional objectives, regain independence, and ultimately enhance their overall quality of life through personalized therapy sessions.

• A Bachelor’s or higher degree in Physical Therapy from an accredited institution is required.
• A current and valid Physical Therapy license in New Jersey is mandatory.
• At least 1-2 years of proven experience in physical therapy, preferably in a homecare or outpatient environment.
• Exceptional clinical assessment skills and proficient manual therapy techniques with a solid grasp of physical rehabilitation methodologies.
• CPR and First Aid certification is preferred; additional qualifications in geriatrics or neurological rehabilitation would be advantageous.
• A valid driver's license along with reliable transportation is required for patient home visits.

• Experience in working with elderly patients or those recovering from orthopedic surgeries is beneficial.
• Familiarity with electronic health records (EHR) systems and documentation protocols is an asset.
• Proven track record of developing effective therapy programs tailored to individual patient needs.

Mend Force Physical Therapy offers a competitive salary range of $110,000 to $120,000 annually, reflecting the experience and qualifications of the successful candidate.
