Position Summary:

Unison Health is seeking a Case Manager to join our Home-Based Treatment (IHBT) team, providing services to youth and families with complex behavioral health needs. This role is ideal for professionals who value flexible, community-based work, small caseloads, and the opportunity to make a long-term impact through short-term, intensive support.

Key Responsibilities & Role Highlights:

  • Small caseload – serve a limited number of youth and families at a time

  • Flexible, community-based hours – appointments take place in the home or community

  • Team-based, collaborative work environment

  • Extensive training and professional development opportunities

  • Support for career growth, including supervision for those pursuing licensure

  • Participation in 24/7 on-call rotation, with premium call pay included

  • Time-limited, intensive family support with long-term positive outcomes

  • Deliver intensive home-based case management services in accordance with state mandates and agency policies

  • Collaborate with clinical therapists to support individualized family service plans

  • Document all services provided and maintain accurate client records in the electronic health record system

  • Ensure compliance with confidentiality guidelines and proper handling of release of information

  • Participate in 24/7 on-call rotation as required

Education & Experience Requirements:

  • Minimum of a bachelor’s degree in Social Work, Counseling, Psychology, or a related field

  • Must meet Ohio MHAS criteria for Qualified Mental Health Specialist (QMHS) if not licensed

  • Valid driver’s license and reliable transportation required

  • Must be insurable under the agency’s commercial liability policy

  • Experience working with children and families preferred

  • Must demonstrate knowledge in:

    • Mental illness and treatment

    • Individual and group counseling

    • Behavioral health assessments and crisis intervention
