Work Environment

This role involves working onsite, primarily in the patient's home environment. As a Home Care Physical Therapist, you will have the unique opportunity to engage with patients in the comfort of their own homes, which allows for a tailored approach to rehabilitation and recovery. You'll work standard hours from Monday to Friday, offering flexibility in managing your schedule to best meet patient needs.

Key Responsibilities

• Conduct comprehensive evaluations of patients' physical abilities and limitations in a home setting, taking into account their individual conditions.
• Develop and implement customized treatment plans that incorporate various therapeutic methods such as therapeutic exercises, manual therapy, and use of specialized equipment.
• Monitor patient progress, making adjustments to treatment strategies as needed to ensure optimal outcomes.
• Document all treatments and patient responses meticulously in compliance with healthcare regulations, using our electronic health record system.
• Collaborate with caregivers and families, providing training on exercises and assistive devices to foster a supportive recovery environment.
• Stay updated on best practices in physical therapy through continued education opportunities, workshops, and networking with peers in the field.
• Reassess and adapt therapeutic goals regularly to align with patient progress and needs.
• Advise on the provision and fitting of assistive devices, when necessary, to aid self-care and mobility.

Patient Population / Client Focus

You will work with a diverse patient population, covering a wide range of ages and medical conditions. This can include individuals recovering from surgeries, injuries, or chronic illness. Your role is crucial in restoring their independence and enhancing their quality of life through dedicated, in-home rehabilitation services.

Qualifications

• A Bachelor’s Degree in Physical Therapy, with a preference for candidates holding a Doctorate in Physical Therapy (DPT).
• A valid state license to practice Physical Therapy in Virginia.
• Exceptional communication skills, both written and verbal, to effectively engage with patients and their families.
• Proficiency in basic computer skills and electronic health record systems.
• A passion for patient care and a commitment to helping others achieve their rehabilitation goals.
• Ability to adapt quickly to varying patient environments and needs.

Preferred Experience

• Minimum of 2 years of experience in a Physical Therapy role, particularly in home health settings.
• Experience with utilizing modern therapeutic modalities and equipment.
