Reliable Home Care Providers, Inc. is currently seeking a licensed Physical Therapist to join our growing team in a part-time, field-based capacity. This opportunity is well-suited for a clinician looking to build their patient load gradually while maintaining flexibility over their own schedule. The position involves providing one-on-one, skilled therapy in the comfort of the patient's home.

Primary Responsibilities:

  • Conduct in-home physical therapy evaluations and develop individualized treatment plans
  • Deliver skilled physical therapy services aimed at improving mobility, strength, balance, and safety
  • Educate patients and caregivers on home exercise programs and injury prevention
  • Monitor patient progress and adjust care plans accordingly
  • Coordinate with physicians, nurses, and care managers to ensure seamless care
  • Complete accurate, timely clinical documentation in accordance with state and federal regulations
Requirements

Preferred Candidate:

  • Licensed Physical Therapist in the State of Illinois (required)
  • Minimum of 1 year of experience in home health or adult rehab setting preferred
  • Working knowledge of electronic documentation systems (e.g., Axxess, Kinnser/WellSky, etc.)
  • Strong time management, organizational, and interpersonal skills
  • Must be self-motivated, professional, and reliable
  • Valid driver's license, reliable transportation, and current auto insurance are required
