Job description

Sabathani Community Center (SCC) is one of Minnesota's oldest African American-founded nonprofits & serves a multi-cultural community of more than 50,000 community members in South Minneapolis. SCC was founded in 1966 by a handful of concerned community members seeking to address social and economic disparities in underserved communities in the areas of youth programming, housing, health and education. About the role
Sabathani Community Center's Health Equity Department works to reduce health disparities and improve access to care for community members through direct service, care coordination, and community education. The department's programming is designed to meet residents where they are, with a focus on culturally responsive, trauma-informed care.
Sabathani Community Center is seeking a Family Home Visitor to provide individualized, in-home support to low-income families with young children or who are expecting a child. The Family Home Visitor carries a caseload of up to 25 families, building trust-based relationships through regular home visits centered on parenting education, child development, family stability, and connection to community resources. This role is grounded in a strengths-based, trauma-informed, and culturally responsive approach, and is well suited to someone who is comfortable building relationships in the community and holding space for families navigating complex needs.
The physical duties associated with this position are consistent with duties associated with work performed in an office and community setting, including home visits. This job description serves only as a general description of the anticipated day-to-day responsibilities of the position. To perform this job successfully, you must be able to carry out each essential job function satisfactorily.
What you'll do
Home Visiting & Case Management
• Carry a caseload of up to 25 families, conducting a minimum of two home visits per month per family, with increased frequency (up to weekly) during periods of heightened need.
• Conduct home visits lasting approximately 60-90 minutes, held primarily in the family's home with flexibility to meet at Sabathani or another community location based on family preference.
• Maintain ongoing contact with families between visits via phone or text to provide support and follow up on referrals.
• Deliver individualized home visit content including parenting education, child development guidance, parent-child interaction activities, goal-setting and problem-solving support, and safety planning.
• Support family resilience and stress management, drawing on Sabathani's Roots: The Black Family Foundation framework.
Referrals & Resource Navigation
• Connect families to community resources and services including early childhood education, healthcare, housing assistance, food resources, employment support, mental health services, and legal aid.
• Document referrals and follow up to confirm successful connections to services.
• Build and maintain relationships with internal Sabathani programs and external referral partners, including Hennepin County Family Services and Child Protection.
Outreach & Enrollment
• Conduct outreach to recruit eligible families, including engaging community organizations, faith communities, schools, and word-of-mouth referral networks.
• Conduct initial phone or in-person screening to determine potential eligibility based on TANF criteria.
• Support scheduling of the initial assessment visit with the contracted Public Health Nurse.
Group Programming
• Plan and facilitate monthly family workshops at Sabathani Community Center covering topics such as child development, financial literacy, self-care, and community connections.
• Coordinate childcare and light refreshments for workshops to support family participation.
Documentation & Compliance
• Maintain accurate case management records and visit notes for each enrolled family.
• Support TANF eligibility documentation and re-verification in partnership with the Director of Health Equity.
• Track program outputs (visits conducted, families served, workshops held) in a program log.
• Formally update individualized family service plans on a quarterly basis and support quarterly reporting to the Minneapolis Health Department.
Qualifications
Education:
• Bachelor's degree in Social Work, Human Services, Early Childhood, or a related field preferred; equivalent combination of education and experience will be considered.
Experience:
• Minimum 1-2 years of experience in child and family development, home visiting, case management, or a related direct-service role.
• Experience with evidence-based parenting or family support practices preferred.
• Experience working with families facing housing instability, poverty, or system involvement strongly preferred.
• Lived experience and cultural connection to the community served is highly valued.
Knowledge, Skills and Abilities:
• Ability to build trust and maintain strong, culturally responsive relationships with families.
• Working knowledge of child development, trauma-informed care, and family support practices.
• Strong organizational skills with the ability to manage a caseload, meet documentation deadlines, and coordinate across multiple partners.
• Ability to communicate clearly and effectively via oral and written means.
• Ability to maintain appropriate boundaries and exercise sound, independent judgment, including in emotionally sensitive situations.
• Working knowledge of Outlook, Word, and Excel; experience with case management systems a plus.
Licenses and/or Certifications:
• Valid MN Driver's License and access to reliable transportation required for home visits.
• Must pass a criminal background check.
The pay range for this role is:
26 - 28.85 USD per hour (Sabathani-Mpls)
