Primary Care Physician Opportunity - Outpatient Community HealthLocation: Charlestown, NH - Brattleboro 35mEmployment Type: Full-time, permanent positionPosition Type: Employed, Federally Qualified Health Center (FQHC) Position OverviewSeeking a Primary Care Physician to join a collaborative community health team serving patients in southern New Hampshire and nearby Vermont communities. This role focuses on providing comprehensive outpatient care in a patient-centered environment with integrated behavioral health services and strong care coordination support.The position is ideal for physicians who value team-based care, evidence-based medicine, and developing long-term relationships with patients while managing a diverse and sometimes complex patient population.
