• Act as a vital liaison between clients, their families, and outpatient providers to ensure comprehensive care coordination.
• Collaborate with insurance companies for pre-certification processes and utilization management, advocating for patient needs.
• Participate actively in discharge planning and aftercare planning discussions, ensuring a smooth transition for clients post-treatment.
• Provide therapeutic meal support, demonstrating and promoting healthy relationships with food as part of holistic wellness.
• Deliver empathetic and affirming care tailored to each client’s intersecting identities, fostering trust and engagement in the therapeutic process.

This position primarily supports individuals undergoing detoxification, a critical phase of recovery from substance use disorders. The LCSW will work with a diverse population, including adults from various backgrounds and age groups, focusing on understanding and addressing their unique psychological, emotional, and social needs during this challenging time.

• Master's degree in Social Work (MSW) from an accredited institution.
• A valid Licensed Clinical Social Worker (LCSW) license in the state of New York.
• Strong clinical and diagnostic skills, reflecting the ability to assess and treat complex mental health issues.
• Exceptional communication and interpersonal skills are essential for engaging effectively with clients and their support systems.
