Registered Nurse Case Manager

Registered Nurses, in our Case Management team, provide more than just medical assistance; they also listen attentively to patients' needs and honor their wishes. One of the most important aspects of being a Registered Nurse Case Manager is building trusting relationships with patients and their families. This trust allows us to serve as the central point of communication within our interdisciplinary team of holistic care providers.

Hospice nurses play a vital role in helping patients spend their final days in a way that truly reflects who each individual is and what matters to them most. More than providing medical care, it's about going above and beyond to honor their wishes and ensure they have dignity and comfort. This requires a deeper level of commitment and compassion, but the impact is immeasurable. Every day, we have the opportunity to create meaningful moments for patients and their families, knowing that we are helping them navigate one of life's most difficult journeys.

RN - Registered Nurse Qualifications:
  • 6 months minimum prior skilled nursing experience
  • An active unencumbered state Registered Nurse license
  • Current driver's license and auto liability coverage
  • Interest and ability in providing patient care in homes and facilities
RN - Registered Nurse Benefits & Schedule:
  • Schedule: Monday to Friday, 8:30 AM - 5:00 PM
  • No Mandatory On-Call Required
