ORGANIZATION OVERVIEW:ย ย 
Face to Face Health and Counseling is a non-profit organization dedicated to advancing economic and health equity for youth ages 11-24. We do this work through a comprehensive approach that includes housing, mental health, medical, youth justice, education & employment, and community services. We take a strengths-based and trauma-informed approach to our work and center the needs of young people impacted by racism and other forms of oppression and marginalization. Founded in 1972, Face to Face serves more than 3,000 youth annually.


JOB SUMMARY:ย 
The Housing Coordinator is responsible for leading the process for youth to access safe affordable scattered site housing, manage on-going rental assistance, ensure grant/contract compliance, and HMIS data quality and reporting.ย 


RESPONSIBILITIES:

Coordinated Entry Liaison

Primary contact between Face to Face Housing programs and Coordinated Entry (CE);

Request youth from the priority list to fill openings based on funding stream;

Review documentation to ensure youth meet eligibility and compliance prior to program enrollment;ย 

Complete necessary HMIS forms to enroll youth into Face to Face housing;ย 

Contact youth and complete Face to Face intake and/or enrollment to the housing program;

Coordinate with housing case manager(s) for housing search;

Follow policy and procedure to return CE referrals when youth are unreachable/unresponsive;

Keep track of the housing program vacancy rate to project timely move-in/move-out status of youth;

Coordinate with the Director of Youth and Family Services to ensure CE requests and move-in/move-out status of youth is aligned with grant work plans/budgets.

HMISย 

Perform regular and timely data entry of client and service data into HMIS;

Ensure all HMIS compliance is complete for housing funding streams;

Ensure completion of necessary reporting from HMIS for timely grant compliance and agency statistical analysis;

Train direct service staff and management to work with the HMIS data tracking system;

Work with direct service staff to correct missing or conflicting information;

Serve as the HMIS expert to resolve issues that come up with HMIS data and propose process improvements to reduce data error;

Keep up-to-date on HMIS data standards and changes for all Face to Face programming and assist leadership in implementation of those changes;

Attend funder meetings related to data and assist Face to Face in meeting funder data requirements;

Work with Agency Data Specialist to ensure consistent data standards and collaboration across program areas;

Provide regular reports for housing leadership and staff on program outcomes and demographics;

Develop competence in other Face to Face data management systems;

Rental Assistance

Ensure on-going rental assistance compliance by securing copies of leases, and on-going monthly ledgers from landlords;

Ensure youth rental assistance funds are aligned between HMIS and Finance;

Update the rent tracker monthly;

Submit monthly rental assistance requests to the Director of Youth and Family Services for youth enrolled in the housing program;

Coordinate with the Director of Youth and Family Services to ensure rental assistance management/projections are aligned with program/grant budgets.

Update Apricot to reflect rental assistance support services;

Other

Provides all services guided by the evidence-based principles for helping youth overcome homelessness; and

Perform other duties as assigned.


QUALIFICATIONS:

Minimum of 2 years' experience working with the homeless or at-risk youth;

Experience with a positive youth development and harm reduction approach;

Minimum of 1-2 years' experience working with data management systems, HMIS preferred;

Proficiency with Microsoft Office products;

Excellent analytical and problem-solving skills;

High level of accuracy and attention to detail;

Ability to manage time well, prioritize efficiently and meet competing deadlines;

Flexibility to respond to changing priorities;

Personable and able to work well with direct services staff and leadership;

Excellent interpersonal, verbal and written communication skills;

Ability to work both independently and as part of a team;

Ability to maintain confidentiality and exercise judgment over the treatment of sensitive issues; and

Demonstrated ability to work and communicate effectively with people of diverse ethnic, economic, racial and cultural backgrounds.

LOCATION: Essential Onsite Flex - This role requires the employee to be onsite, with the possibility of some specific and limited remote hours. Employees are required to be fully onsite during the initial 3-month training period.

PAY & BENEFITS:ย The pay rate for this position is between$24 and $26, based on qualifications.ย 

Face to Face offers benefits which include health, dental, short-term disability, long-term disability, life insurance, 403b account (3% employer match) as well as 23 days of PTO and 11 paid holidays.ย
