Note: Positions at 48th Ave. are expected to end 12/31/2026.

POSITION SUMMARY

The Case Manager (CM) should be someone who takes initiative, can multi-task, and is able to manage a large caseload. The CM provides assessments, assignments, and accountability to guests and participants in our facilities and programs.


RESPONSIBILITIES

  • Maintains the relationships necessary in community and across DRM to ensure DRM's case management efforts reflect a real-time understanding of broader re-housing efforts, availability, requirements, and protocols.
  • Demonstrates respect, cultural competence, and knowledge of trauma-informed care in interactions with participants, staff, and other community providers.
  • Develops a case plan with individualized goals based on individual needs and facility/program requirements.
  • Identifies action steps necessary to achieve goals, and monitors progress through scheduled follow up meetings.
  • Maintains professional best practices in areas of document storage, confidentiality and professional ethics when dealing with participants and sensitive information.
  • Regularly updates all case planning records, including 22 Things, case notes and case plans in SCRIBE including uploading vital documents and ROIs.
  • Documents all meetings, referrals, and services in SCRIBE and utilizes the appropriate templates.
  • Provides SCRIBE reports, spreadsheets and statistics as requested.
  • Engages in ongoing training as assigned by the direct supervisor and leadership team.
  • Seeks to develop a continuing knowledge of the field, what services are provided in the community, and establishing communication with those entities.
  • Collaborates with other DRM personnel regarding participant evaluation, goals and continuity of care.
  • Facilitates Community Provider visits with participants as assigned, including signing up participants to meet with Community Providers and documenting communications.
  • Upholds all residential and DRM program policies and written agreements.
  • Monitors, schedules, and conducts case planning meetings to make sure a maximum number of appointments are offered.
  • Documents all meetings and required services in HMIS/Clarity and maintains participant records, including uploading vital documents and ROIs.
  • Attend and engage in weekly and monthly meetings as assigned.
  • Collaborates, communicates, and advocates with service providers in the government, medical, mental health, housing, benefits acquisition, employment, legal, and veterans, etc. fields to aid participants toward a successful transition into long-term programs or sustainable housing.
  • Other duties as assigned by direct supervisor or leadership team.

WORKING CONDITIONS

  • Occasional lifting up to 35 lbs., bending, and climbing stairs.
  • Working with individuals who are experiencing homelessness, addiction, mental illness, and a myriad of other issues related to homeless populations.
  • Possible exposure to communicable diseases.

Requirements

MINIMUM QUALIFICATIONS

  • Bachelor's degree OR two years of related work experience required.
  • A combination of education and experience may be substituted.
  • Colorado Certified Addictions Specialist (CAS) certification or equivalent preferred.
  • Active Driver's License as a limited amount of work-related travel may be required.
  • Ability to obtain and maintain CPR certification within 90 days of hire.
  • Must be a Christian holding to the beliefs set forth in the Denver Rescue Mission Statement of Faith, expressed by a personal testimony and Christian conduct.
  • Excellent verbal and written communication skills, and responsiveness to communication.
  • Ability to resolve conflict professionally and respectfully while maintaining composure under pressure.
  • Strong working knowledge of Windows software applications and ability to learn SCRIBE and Clarity systems.
  • Ability to follow highly detailed workflows.
  • Willingness to possibly work weekends and travel between locations.
