We are currently seekingan RN Clinical Liaison at Froedtert Hospital, Pleasant Prairie Campus and throughout the Kenosha County community who will represent Horizon by helping patients and families make a smooth transition into hospice. Self-starter, multi-tasking master will:

  • Serve as a liaison between the patient and family, the Agency, and member hospital.Develop ongoing relationships with referral contacts, respond to referrals in a timely manner and develop a plan to support growth.
  • Communicate and collaborate with the Hospice Team regarding admission information and any changes to the patient status that could impact the patient admission process.
  • Complete admissions: Conduct thorough patient assessment that allows for the development of a transition plan and plan of care that will ensure comfort, safety, and peace of mind.
  • Interact and collaborates with patient, patient family and/or significate caregivers to coordinate referral to services.
  • Ensure goals are clearly delineated and prioritized on the patient plan of care and addressed by all team members
  • Ensure plan of care reflects assessment findings to establish appropriate goals / interventions to meet patient needs and that goals are realistic and measurable.
  • Case conference with team members occurs regularly which includes creating PCW / HHA Care Plans, initiating and updating Plans in accordance with patient needs and Agency policy.
  • Inform physician, clinical manager and interdisciplinary team members of changes in patient status as evidenced by Case Communication documentation.
  • Assure appropriate discharge planning occurs from the start of care by utilizing a multidisciplinary approach and appropriate documentation.
