Job Overview:
St. Mary’s Alexandria is seeking compassionate and skilled Direct Care Professionals (DCPs) to provide in-home support and care for our clients. The DCP is responsible for managing, supervising, and delivering direct care to clients in their homes, ensuring each individual receives personalized support in line with their Individual Habilitative Program (IHP).

Key Responsibilities:

  • Provide direct care and supervision to clients in their homes, promoting independence and well-being.
  • Implement Individual Habilitative Program plans tailored to each client’s needs.
  • Administer interventions to manage inappropriate behaviors, ensuring safety and positive outcomes.
  • Maintain detailed documentation of care provided and client progress.
  • Collaborate with families and team members to ensure consistent, high-quality support.

Qualifications:

  • Previous experience in direct care, residential care, or home health preferred.
  • Strong communication and interpersonal skills.
  • Ability to implement individualized care plans and behavioral interventions.
