Make a Difference in the Lives of Seniors

The Salvation Army of Syracuse is seeking a compassionate and dedicated Neighborhood Advisor to support adults age 60+ through outreach case management, housing crisis intervention, advocacy, and supportive services.

In this rewarding role, you will help seniors maintain safe, stable, and healthy living environments while working to prevent homelessness and unnecessary hospitalization.

  • Provide outreach-based case management and supportive services
  • Assist seniors with housing stabilization and crisis intervention
  • Advocate for clients’ needs and connect them with community resources
  • Support individuals and families in maintaining safe, affordable housing

 Responsibilities
  • Maintain accurate client records, including demographics, services provided, outcomes, and progress notes
  • Conduct presentations at local community centers to connect seniors with available services and resources
  • Enter presentation and client data as required by funding agencies
  • Submit reports and documentation to supervisors and funders in a timely manner
  • Develop and maintain strong community partnerships and referral networks
  • Utilize agency-issued technology to manage client information and case documentation
  • Maintain regular contact with clients through home and community visits using a personal vehicle
  • Coordinate services with community providers and advocate for client needs
  • Support seniors seeking assistance at the Adult Day Program Center
  • Assist clients with housing searches, landlord communication, and housing resource navigation
  • Provide information and referrals to adults age 60+
  • Conduct outreach through door-to-door engagement, referrals, and community presentations
  • Attend required agency and funder trainings
  • Follow up on referrals to ensure seniors successfully access services
  • Work independently while managing schedules, priorities, and documentation efficiently
  • Demonstrate cultural competence and support the mission and strategic goals of The Salvation Army Syracuse Area Services

Qualifications

Education

  • Associate degree or technical school certification 

Experience

  • 3-5 years experience in geriatric casework, outreach case management, social services, or community-based programs preferred
  • Ability to thrive in a fast-paced, ever-changing environment with shifting priorities and frequent redirection

Computer & Technology Skills

  • Proficiency in Microsoft Outlook and Microsoft Office Suite
  • Comfortable using technology in the field to complete documentation, communication, scheduling, and data entry in a timely manner
