Heartland Home Health is seeking a Patient Care Manager (RN Clinical Manager) to lead clinical operations and ensure excellent patient outcomes through effective care planning, team collaboration, and regulatory compliance in our Milwaukee home health branch.

Essential Functions
  • Report directly to the Executive Director or Administrator, overseeing the daily clinical operations of your assigned site.
  • Ensure individualized, appropriate, and up-to-date Plans of Care for all patients, adjusting for significant changes in condition, hospital discharges, or physician updates.
  • Serve as a liaison between referral sources, patients, caregivers, and your team to facilitate seamless care coordination.
  • Oversee all clinical, administrative, and recordkeeping functions, ensuring compliance and accuracy.
  • Provide hands-on skilled nursing care in the home when needed and participate in the on-call rotation.
  • Support and guide your clinical team through coaching, performance management, and collaboration.
  • Step into the site leadership role during the absence of the Executive Director or Administrator.
  • Help build and maintain a positive and engaged clinical culture that prioritizes patient-centered care and regulatory excellence.
About You

Qualifications - What You'll Bring:

  • Associate's degree in Nursing from an accredited program (bachelor's degree preferred).
  • Current RN license in the state of employment.
  • CPR certification required.
  • Minimum 3 years of clinical nursing experience, preferably in home health or a related setting.
  • Strong clinical judgment and critical thinking skills.
  • Ability to lead teams, manage schedules, resolve issues, and support quality improvement.
  • Proficient in nursing assessment tools and comfortable with electronic documentation systems.
  • Valid driver's license, auto insurance, and reliable transportation.

Preferred Experience (Not Required):

  • Clinical team leadership or supervisory experience
  • Home health, hospice, or case management background
  • Experience navigating state and federal healthcare regulations
  • Knowledge of EMRs (Homecare Homebase) and scheduling platforms
