Job description

Physical Therapist (PT) – Home Health Care

Pay: $80 per visit

Location: O‘ahu, HI

Schedule: Flexible PRN Schedule

Always Best Care Home Health helps seniors and kupuna remain independent while receiving care in the comfort of their own homes. Our clinicians work closely with patients and families to improve mobility, reduce fall risks, and support overall well-being.

We are looking for a Physical Therapist who is passionate about patient-centered care and enjoys working one-on-one with patients in a home setting.

What You'll Do
  • Conduct physical therapy evaluations and reassessments
  • Create individualized treatment plans
  • Address mobility, balance, strength, and safety concerns
  • Provide patient and caregiver education
  • Monitor progress and adjust treatment plans as appropriate
  • Coordinate care with the interdisciplinary team
  • Complete Medicare-compliant documentation
Requirements
  • Hawaii Physical Therapist license
  • Minimum one year of clinical experience
  • Home health experience preferred
  • Reliable transportation
  • Strong communication and organizational skills
