Job description

POSITION SUMMARY:
  • The essential functions of the job for the LPN, requires, and may be amended from time to time at the sole discretion of Viva Senior Living to include, but are not limited to the following:
  • Comply with standards of business conduct in accordance with federal, state and local health and regulatory standards and guidelines, as applicable.
  • Must possess a valid LPN license issued by the State School of Nursing
  • Must possess, at a minimum, a high school diploma or a GED
  • Must be able to work well with peers in a congenial work environment, and above all, maintain the utmost compassion and dignity to the residents and adhere to the Resident Bill of Rights.
  • In exigent circumstances, the employee may be subject to call back during emergency conditions (e.g., severe weather, evacuation, temporary staffing shortages, etc.).
  • Must adhere to Viva Senior Living’s attendance policies and follow the terms and conditions of employment as set forth in the Employee Handbook.
  • Must be able to read, write, speak and understand English.
  • Must maintain the highest standards in caring and servicing the needs of the residents and residents' family members and loves ones.
  • Must adhere to all facility policies and procedures.
RESPONSIBILITIES/ACCOUNTABILITIES
  • The Wellness Nurse is responsible to provide care, treatment, medication administration to assigned residents, and to continually monitor and assess the residents’ condition and report in accordance with plans of care, established medical practices, and all applicable laws and regulations, and Viva’s policy and procedure.
  • Assists with oversight of ancillary care staff in the absence of the Wellness Director or Assistant Wellness Director.
  • Provide care to assigned residents based on plans of care
  • Medication assistance and/or administration with adherence to the Five Rights of Medication Administration
  • Reordering and receipt of medications
  • Provide personal care and ADL assistance
  • Supervision of Care staff
  • Maintains shift to shift report
  • Provides emergency assistance to residents with appropriate follow-up
  • Obtains and transcribes physician orders
  • Maintains case management documentation
