Job description

About Care Continuity
Care Continuity is redefining patient navigation. We combine clinical expertise, AI-driven insights, and compassionate human support to ensure patients receive the care they need, when and where they need it. Our solutions empower health systems and providers to close care gaps, reduce readmissions, and drive ROI through smarter, more connected navigation.
Our work is transforming how care decisions are made, and we're just getting started.
Role Overview
As a Healthcare Data Analyst, you will help healthcare organizations unlock the value of their clinical and operational data.
Initially focused on one of Care Continuity's largest health system partners, you will analyze patient movement and care utilization patterns using Health Information Exchange (HIE), ADT, referral, and clinical data. Your work will help identify opportunities to improve patient navigation, strengthen network integrity, reduce care leakage, and drive growth for both Care Continuity and our customers.
This role combines healthcare analytics, data investigation, customer collaboration, and business insight generation. You will work closely with product, analytics engineering, customer success, and health system stakeholders to transform complex healthcare data into actionable recommendations and compelling stories.
The ideal candidate is highly analytical, comfortable working with messy healthcare datasets, and curious about how data can reveal opportunities to improve healthcare delivery and patient outcomes.
What You'll Do
  • Analyze ADT, CCD, and other healthcare datasets to identify trends, opportunities, and operational insights
  • Build and maintain a living inventory of available data feeds, fields, and coverage across facilities - identifying gaps, surfacing underused data we already receive, and preventing duplicate or missed integration work.
  • Investigate data quality issues and collaborate with internal and customer teams to improve data completeness, accuracy, and usability
  • Perform exploratory analysis to uncover opportunities related to patient navigation, care coordination, network integrity, referral management, and patient retention
  • Develop reports, dashboards, and presentations that communicate findings clearly to healthcare leaders and operational stakeholders
  • Support customer implementations by validating incoming healthcare data and assisting with data mapping activities
  • Work with analytics and data engineers to build and enhance the data pipelines that feed BI reporting, and take ownership of the reporting-layer datasets and semantic models that sit on top of them.
  • Own the end-to-end path of getting validated data into our BI/reporting environment - defining data models, building and maintaining datasets, and ensuring published reports stay accurate as feeds change.
  • Investigate interface and data flow issues across ADT, CCD, and other feeds
  • Partner with customer success and product teams to translate analytical findings into actionable recommendations
  • Help quantify business impact and ROI opportunities for health system customers
  • Support development of new analytics offerings, dashboards, and insight-generation capabilities
  • Document data assumptions, business rules, and analytical methodologies

What We're Looking For
  • 3-7 years of experience in healthcare analytics, healthcare data management, population health analytics, clinical informatics, or a related field
  • Strong SQL skills and ability to work independently with large datasets
  • Experience analyzing healthcare data including ADT, HIE, claims, referral, clinical, or encounter data
  • Hands-on experience with healthcare interoperability standards including HL7, ADT feeds, and CCDs (FHIR a plus), including the ability to read message structures, troubleshoot interface issues, and validate that incoming data maps correctly
  • Ability to investigate and troubleshoot data quality, mapping, and interface issues
  • Strong analytical and problem-solving skills with attention to detail
  • Ability to communicate complex findings to both technical and non-technical audiences
  • Experience developing dashboards, reports, and executive-facing presentations
  • Comfort working directly with healthcare stakeholders and customers
  • Curiosity, initiative, and a passion for using data to improve healthcare delivery
  • Strong Power BI experience - building and maintaining datasets and data models, not just visuals - with the ability to publish and maintain reports that stay accurate as underlying feeds change.

Nice to Have
  • Experience implementing or supporting Health Information Exchanges (HIEs)
  • Experience with provider network analysis, referral analytics, care coordination, or patient navigation programs
  • Experience working with health systems, physician groups, or value-based care organizations
  • Experience supporting customer-facing analytics engagements
  • Background in health tech, population health, care management, or digital health

Salary & Benefits
  • Salary range: $100,000 - $130,000 annually, depending on experience and location
  • Comprehensive benefits package, including medical, dental, vision, and 401(k)
  • Equity opportunities
  • Flexible PTO and fully remote work environment
