• Conduct thorough and ongoing assessments of patients to tailor services that meet their specific mental health needs.
• Maintain open lines of communication with supervisors, families, caregivers, and other healthcare professionals to provide updates on patient progress and treatment plans.
• Prepare and submit a detailed schedule to supervisors that includes appointments, home visits, and relevant observations to monitor patient advancement.
• Coordinate home visits for personal assessments, chart reviews, and interviews, while analyzing data to gain insights into patient needs.
• Implement and oversee the effectiveness of treatment program plans, ensuring treatment strategies are properly executed.
• Participate proactively in Interdisciplinary Team Meetings and contribute to discussions that shape patient care approaches.
• Document and complete monthly and quarterly progress notes that track patient advancements and areas needing attention.
• Organize and conduct training sessions to certify that staff and team members are well-versed in treatment implementation and documentation requirements.
• Provide follow-up interviews, home visits, and ongoing observations to support patients in their treatment journey.
• Ensure that all records are accurately maintained and easily accessible to authorized individuals.
• Act as a liaison with both the public and the mental health community, fostering partnerships to advance community health initiatives.
• Assist in developing and refining training materials and procedures to continually improve service programs for individuals.

As a Licensed Professional Counselor at Crimson Care Center, you will engage with a diverse clientele, including both adults and youth facing various mental health challenges. Our practice emphasizes personalized approaches based on each individual’s unique circumstances, whether they are grappling with anxiety, depression, trauma, or relationship issues. The role is pivotal in helping clients transition towards healthier, more fulfilling lifestyles through tailored mental health support.

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.
• You must possess an active and unrestricted Professional Counselor (LPC) license for the state of Texas.
• A minimum of two years of counseling experience in a clinical setting is necessary to ensure competency in delivering effective care.
• Specializations in family therapy, couples therapy, trauma, or substance misuse are highly desirable.
• Strong knowledge of ethical and legal standards in mental health practice is essential.
• Excellent interpersonal and communication skills are required to build rapport with clients from diverse backgrounds.
• Proven ability to work collaboratively within a healthcare team is expected.
• Comfortable using EMR software and technology to document patient care and progress.

• Experience working with specific populations, including children and adolescents, is advantageous.
• Demonstrated previous success in developing treatment plans and implementing therapy sessions tailored to individual client needs is preferred.
• Familiarity with community resources and mental health networks in the Houston area is a plus.
