The Care Manager will be responsible for providing patient centered, strengths-based care management services to ensure outreach, engagement, assessment, care planning development & implementation, coordination and monitoring of assigned patients living with HIV. Through a collaborative effort with the patient’s care team, the Care Manager will coordinate and advocate for services that will promote optimal health outcomes and reduce the usage of the Emergency Department and inpatient hospital stays.


  • Bachelor’s Degree in Human Services/Social Work or related field required.
  • 3 minimum of 3 years case management experience in a health care setting, HIV/AIDS and/or alcohol/substance use program required.
  • CASAC or Qualified Health Professional preferred.
  • Bilingual (English/Spanish) required.
  • Valid drivers license required.
  • Access to personal car for work related travel required.
  • Proficient computer skills.
  • Possesses well-developed and effective interpersonal skills and can communicate effectively verbally and in writing.
