Requirements:
- Current state license as a Registered Nurse
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years Home health experience

Roles & Responsibilities:
- Administer skilled nursing care to clients of all ages in their place of residence.
- Assess patients' physical, psychosocial, and environmental needs in both stable and emergent situations.
- Develop, implement, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and families on care, medications, and home safety practices.
- Supervise and provide clinical direction to support staff to ensure quality service.
- Monitor and reassess patients' pain management needs and provide appropriate interventions.
